The Clinical Query limits allow you to restrict the results of a Medline search to certain types of studies. For instance, if you searched for the MeSH heading ASTHMA/ and then wanted to limit the results to therapeutic studies, you could click on the Limit icon and choose one of the Therapy limits.
Each Clinical Query limit has three different versions: one that maximises sensitivity (so your search results might include some irrelevant studies but are more likely to be comprehensive); one that maximises specificity (so your final results might exclude some relevant studies but are more likely to include the very relevant ones); and one that optimises the trade-off between sensitivity and specificity.
